Home Health Aides or Personal Care Aides provide companionship, safety supervision, personal care, and light housekeeping services under the direction of a Registered Nurse. Additionally, aides can accompany clients to physician offices or to other scheduled appointments.

Aides will assist with:

• Personal grooming (bathing)
• Ambulation
• Prepare Meals
• Shop and run errands
• Accompany to appointments
• Provide safety supervision
• Perform light housekeeping

Nursing Supervision to include:

Visits as needed for the development and monitoring of a plan of care
Monitoring of the home care worker’s performance
Crisis intervention, if needed
Arrange for consultations and/or referrals as needed

Case Management

A Registered Nurse will conduct in-home assessments, provide health information, work closely with personal physicians, provide supervisory home visits, consultations and will refer patients to specialized services, as needed.
